Job Summary
The Outreach Worker plays a crucial role in connecting individuals with community resources and support services. They work directly with underserved populations to provide guidance, support, and advocacy, ensuring that community members receive the assistance they need to improve their quality of life.
Key Responsibilities
- Engage with community members to identify their needs and provide information about available resources.
- Coordinate with local organizations and service providers to facilitate access to services.

Role and Responsibilities
1. Community Engagement
- Engage with individuals in the community to understand their needs, challenges, and potential barriers to accessing services.
- Build and maintain trust with underserved populations to encourage participation in available programs.
2. Support Services
- Provide information and referrals to social services, healthcare, housing, and employment opportunities.
- Assist clients with completing paperwork, applications, and navigating bureaucratic systems to access services.
3. Crisis Intervention
- Respond to emergency situations, such as homelessness, domestic violence, or substance abuse, providing immediate support and connecting clients to crisis intervention services.
- Coordinate with other service providers to ensure the individual receives comprehensive support.
4. Advocacy
- Act as an advocate for individuals by helping them voice their needs and ensuring they receive appropriate assistance.
- Raise awareness of issues affecting the community and contribute to community development initiatives.
5. Follow-Up & Monitoring
- Maintain ongoing contact with clients to monitor their progress and offer continuous support.
- Track the outcomes of outreach efforts and report on the effectiveness of programs.
6. Collaboration with Agencies
- Work closely with government agencies, non-profits, healthcare providers, and other community-based organizations to ensure individuals receive holistic care.
- Attend community meetings and collaborate with stakeholders to create and improve service delivery.
7. Record-Keeping & Documentation
- Maintain accurate records of client interactions, referrals, and outcomes in a secure database.
- Prepare reports and case notes, ensuring confidentiality and compliance with privacy regulations.
Requirements and Skills
- Education: High school diploma or equivalent (required); Bachelor’s degree in Social Work, Human Services, or a related field (preferred).
- Experience: Experience working in community outreach, social services, or a similar role (required). Experience working with at-risk or underserved populations (preferred).
